Q&A – What caused these patches on my lawn? Is it army worms or herbicide or fertilizer?

What caused these yellow patches on my lawn? Some are large, 4 feet by 2 feet. A lawn company added fertilizer and herbicide about a week before they appeared. Is this fertilizer burn or damage from adding too much herbicide? It looks similar to damage from army worms that I had about 3 years ago. Are the army worms back?
